Have you ever been to an event where they asked you to fill out a form to evaluate what happened? What questions were unique that could be applied to most events beyond the, Did you have fun?

I think I have experienced this before. An international non-government organization that I connected with usually asks for an evaluation of an event that happened.
I am an event organiser and after the event both my husband and I would relive the moment.
