I prefer to hear the game as it was intended to be played, so very rarely..
Occasionally if there's a "grindy" trophy or achievement I'm gunning for, I'll stick some music on just to try and lose some of the monotony of the task at hand.. ?
If I can realistically save the person experiencing a medical emergency a ride, then so be it.
Otherwise I would pull over and support both my friend and the love of my life with the medical emergency so as not to leave them on their own.

A couple of months ago at work, I was moving the milk delivery from the truck to the warehouse. I got to the chiller, tried to pull the cage sideways (it usually works), but it fell over and hit my leg.
Thankfully it wasn't any more than a bit of a cut.
